These policies will be followed when your pet is being boarded to insure the health and well-being of all. 1. All dogs and cats must be current on their vaccinations. Proof of the following must be provided in written form before or at the time of check-in: Dogs: Rabies, Distemper/parvo, & Bordetella Cats: Rabies & Distemper 2. Canine classic Spa & resort reserves the right to prohibit admittance of pets lacking proof of vaccinations and/or pets displaying signs of untreated or potentially contagious conditions. 3. If external or internal parasites are found, these will be treated to prevent exposure to others. Owners will be responsible for treatment costs. 4. Canine Classic Spa & Resort is not responsible for lost or damaged personal belongings. 5. Any pet that is left unclaimed for 4 weeks beyond the specified check-out date, will be considered abandoned and will be handled according to Illinois State laws. 6. Owners of a pet that is staying in a suite agree to pay additional night’s fee if the pet causes any damage to the room or furnishings. 7. In the case of emergency we will call you veterinarian. If your vet is not available, Canine Classic Spa & Resort will use one of our on call vets. Owner of pet agrees to be responsible of any all fees . Release Agreement: I am the owner or agent of the pet(s) mentioned on this form and I am authorized to sign this release form. I have read and understand the above terms> I give consent to Canine Classic Spa & Resort to act on my behalf and in my best interest by obtaining veterinary care at my expense if deemed necessary. I hereby assume any expense or liability for injuries or damages my pet may inflict o n a human being, property, or another pet while staying at Canine Classic Spa & Resort.
